I am duly and regularly engaged in the practice of my profession.
I hold a health care professional degree from Sherman College of Chiropractic in Spartanburg, South Carolina.
I have had special professional training and experience in chiropractic forensics, disability evaluation and impairment rating, chiropractic orthopaedics, and independent medical examinations.
I possess a special health care knowledge or skill in chiropractic forensics, chiropractic standard of care, disability and impairment rating. I have successfully completed certification and testing through the American Board of Independent Medical Examiners (ABIME) as a Certified Independent Chiropractic Examiner and the National Association of Disability Examining Professionals (NADEP) as a Certified Disability Examiner, Category I.
I was the author of the 120 hour postgraduate programs in forensics chiropractic and have taught this program throughout the nation in Chicago, IL, Dallas, TX, Las Vegas, NV, Atlanta, GA, Philadelphia, PA, Harrisburg, PA, Miami, FL, and Mt. Pleasant, SC, 1995 through 2001.
I have served as a postgraduate faculty lecturer for impairment rating, disability evaluation, clinical anatomy and special independent medical assessment and orthopedic assessment, Federal Rules of Scientific Evidence, Health Care Law & Ethics, Biomechanical Trauma Master Degree Program, Evaluation and Diagnosis of Mental Disorders, NADEP in cooperation with the University of Florida at Gainesville Health and Science Center, Texas Chiropractic College in Pasadena, TX, Logan College of Chiropractic in Chesterfield, MO, National University of Health Sciences in Lombard, IL, and Lynn University in Boca Raton, FL.

I have authored an accurate and authoritative text and video tape dated January 2001, Medical School for Lawyers Chiropractic Examination and Treatment, Preston B. Fitzgerald, Sr., D.C. and SEAK, Inc., Medical and Legal Information Systems, Falmouth, Massachusetts. This text and video tape addresses chiropractic standard of care evaluation and treatment in spinal trauma. This text and video tape help explain cases involving chiropractic care and clarify what proper chiropractic evaluation, examination, and treatment consists of. This includes the objective and subjective tests, their results, and the significance of these results.
Since 1996 I have provided expert witness opinions in regard to chiropractic evaluation and treatment standard of care for the Pennsylvania Attorney General's Office Insurance Fraud Division.
Since 2001, I serve on the Editorial Advisory Board for Official Disability Guidelines in establishing national standards for disability duration. I have never been disqualified as a medical expert witness in any court in the United States of America.
